1. Services

Shivansh Medical Billing, LLC provides medical billing, coding, claims submission, denial management, and revenue cycle support services to healthcare providers.

2. Eligibility

By using our services, you represent that you are:

  • A licensed healthcare provider or authorized representative

  • Legally permitted to enter into this agreement

3. Client Responsibilities

Clients agree to:

  • Provide accurate and complete information

  • Maintain proper documentation for services billed

  • Ensure compliance with federal and state healthcare laws

  • Promptly respond to billing and documentation requests

4. No Guarantee of Reimbursement

While we strive to maximize reimbursements, we do not guarantee payment or specific reimbursement amounts, as payments depend on payer policies and medical necessity.

5. HIPAA & Compliance

Clients acknowledge that:

  • Shivansh Medical Billing acts as a Business Associate under HIPAA

  • Both parties agree to safeguard PHI

  • A Business Associate Agreement may govern data handling

6. Fees & Payment

Service fees, billing models, and payment terms are outlined in a separate service agreement or proposal. Late or unpaid balances may result in service suspension.

7. Intellectual Property

All website content, logos, text, and materials are the property of Shivansh Medical Billing, LLC and may not be copied or used without written permission.

8. Limitation of Liability

To the maximum extent permitted by law, Shivansh Medical Billing, LLC shall not be liable for:

  • Indirect or consequential damages

  • Lost revenue due to payer decisions

  • Errors caused by inaccurate or incomplete client information

9. Indemnification

Clients agree to indemnify and hold harmless Shivansh Medical Billing, LLC from claims arising out of:

  • Incorrect documentation

  • Non-compliance with healthcare laws

  • Misrepresentation of services provided

10. Termination

Either party may terminate services per the terms outlined in the service agreement. Obligations related to confidentiality and payment survive termination.

11. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to conflict-of-law principles.

12. Changes to Terms

We reserve the right to update these Terms at any time. Continued use of our services constitutes acceptance of the revised Terms.
